Best Practices for Practice Facilitation to support Primary Care Clinics in the HypoPrevent Study

Northwestern University will support development of training and practice including onboarding materials and conducting a “train the trainer” type of session(s) to Avalere staff on best practices to support practices involved in HypoPrevent. Additionally, there is potential for an additional opportunity to have Northwestern support practices in creating a report of patients at risk for hypoglycemia using EHR data once we get rolling.
