Biosense Webster is pleased to inform you that we have reviewed your request for a fellowship grant and our committee has decided to support your Electrophysiology Fellowship Program for the year 2015-2016 in the amount of $50,000.00.
Accompanying this letter is our Fellowship Grant Agreement. The agreement acknowledges that your institution has sole and complete control over the educational nature and content of your fellowship program, including curriculum and course of study, faculty, admittance and completion requirements. Fellowship grant funding is provided consistent with the AdvaMed Code of Ethics on Interactions with Health Care Professionals. Funding support is not intended to induce recipients to purchase or use Biosense Webster products or services.
As a result of the Federal Open Payments ("Sunshine") law (42 C.F.R 403.900), BWI will report to CMS information about payments or transfers of value BWI provides to Institution and/or Fellowship beneficiaries which CMS will make publicly available. The report will include the identity and business address of Institution and/or Fellowship beneficiaries and the value and purpose of any payments or transfers of value that are made. We believe that making this information available to the public demonstrates our commitment to providing the highest quality health care products and services, including support for educational activities. We hope that you share our belief that such disclosure will help the general public to better understand the nature and of educational programs that we support in the interest of advancing patient care.
