This is a patient safety/health care quality program evaluation for Accreditation Council for Graduate Medical Education (ACGME). The project entails a program evaluation of a clinical learning environment/training program at several hospitals (a couple hundred) that is intended, in part, to improve quality and patient safety (and training/education) to do so. For CLER formative site visits are conducted and summarized to provide feedback to address 6 areas: patient safety; health care quality; care transitions; supervision; fatigue management; and professionalism.
