Description
We recently developed a successful model of self-management support for older adults with asthma. The purpose of this R34 application is to advance, adapt and conduct a preliminary test of this care model to support self-management behaviors (SMB) and improve outcomes among people with chronic obstructive pulmonary disease (COPD) in preparation for a future fully powered, pragmatic randomized trial.
