The goal of the Program is not only to deliver physical exercise classes and educational seminars to the Parkinson’s community, though these will be outcomes of the initiative. The Program is additionally intended to help each market build an energetic, welcoming, connected and empowering local community for all individuals with Parkinson’s through: 1. Dialogue and partnership between community-based organizations and medical providers. 2. Education, support, outreach and other wellness programs. Local Jewish Community Centers in the four selected markets will work jointly with NPF Centers of Excellence in each market to deliver the Program. This Letter of Agreement (the “Agreement”) outlines the requirements for participation in the Program for the Northwestern University Parkinson’s Disease and Movement Disorders Center (the “Center”). The Center’s participation in the Program will be to guide the delivery of 36 weeks of wellness programming per year offered at the JCC (defined below), with at least three fitness classes per week and one support group per week. The Center and JCC will collaborate on three community educational symposia each year. Each year over the course of the Program, the total number of individuals participating will increase from the prior year. (The number of classes, support group meetings, symposia, and the goal of increasing numbers of participants are referred to, collectively, as the “Objectives.”)
