This is formative clinical research with focus on the disparities in management of Type 1 and Type 2 Diabetes in Chicago. Spatial analyses of the electronic health records from main health institutions in the area will be leveraged to determine areas in need of improvement and describe the attributes of the relevant communities. For developing how to access the community in need we will leverage patient and clinician engagement expertise from the Patient and Community Advisory Committee of the Chicago Area Patient-Centered Outcomes Research Network as well as Northwestern team of clinicians. The information gathered during this study will enable Chicago Public Health to tailor field operations to their local settings and to identify and address any barriers to operations."
