Description: This team will be responsible to design distress, psychosocial and survivorship support pathways, metrics, identify screening tools and process training to be utilized by pilot and other sites. Objectives of the project: To develop and support pilot of Distress/Psychosocial Oncology and Survivorship care process / pathways to improve delivery of services, patient care processes • Ensure capacity, effectiveness and cost-effectiveness (design and test measures and outcomes of process) • Appropriately identify and serve patient sub-populations based on needs (define processes that are different based on patient needs from screening) • Identify where there are service gaps in Chicago
