EHR-based patient identification, primary care clinician messaging and focused care coordination to reduce delayed diagnosis of colorectal, breast and prostate cancers in patients with abnormal findings

This proposal directly addresses the top priority identified by NMIC of Delayed Diagnosis and Results Management. There is currently a vulnerability in that high-risk patients may not be referred for diagnostic evaluation or may fail to complete the evaluation. There is no safety mechanism in place to follow up when the appropriate plan is not executed.
