EHR-based patient identification, primary care clinician messaging and focused care coordination to reduce delayed diagnosis of colorectal and prostate cancers in patients with abnormal findings

This proposal is to continue our work to address the top priority identified by NMIC of Delayed Diagnosis and Results Management. In year 1 of our work, we delineated the methods to identify thousands of high-risk NMG patients who may not have been referred for diagnostic evaluation or failed to complete the evaluation for possible gastrointestinal malignancy. We are applying for a second year of funding to further test a scalable safety mechanism to follow up when the appropriate plan is not executed and to expand this approach to patients with findings concerning for prostate cancer.
