Description
NIH/NIDDK clinical trial on the effect of a new specific tool (cognitive behavioral therapy) in decreasing diabetes distress. I have attached a document with the NU Involvement description( it pertains to A and B only).
Northwestern Collaborating Role -- focused on patient identification in parts a through b below. Steps c through f are simply provided for reference so that you can see the flow.
a. A member of the Northwestern team will: prepare a list of adult patients with type 1 diabetes, including their contact information (name, address, phone). If their latest HbA1c value is available, that is wonderful to include but not necessary if not easily feasible.
b. Once we reach this step, we will send you a secure email, that you can in turn respond to and attach the electronic file of patient/contact information. We will then send a letter to the patients on the list at the address provided.
c. A letter describing the EMBARK program and study brochure will be sent to each patient on the list Northwestern provides. We have found the letter to be particularly powerful when placed on the letterhead of the organization collaborating (your organization) and signed by a clinician lead. We would administratively of course handle all correspondence and could just place on an electronic version of your letterhead and add your signature (or another individual).
d. The letter describes the EMBARK program along with a brochure we send, and invites individuals to let us know their interest or that they are not interested (by returning a postcard, calling or emailing). If they do not respond within 10 business days, we then contact participants by phone to describe the program and determine interest.
e. Patients that are interested, eligible and enroll in the program will be known to their clinician through a communication through an update as outlined in the study application / consent form.
