Specific aims
Our overall goal is to improve population health outcomes through research on methods for collecting and using PROMs. Our specific aims are to:
- Assess feasibility of using PROMs to engage patients with multiple chronic conditions in target setting, care planning, and monitoring care over time.
- Assess patient and provider views on their experiences of and the acceptability of using PROMs in care delivery.
Results of this project will inform both how PROMs can be effectively incorporated into patient care AND provide a basis for development of performance measures for evaluating health care delivery.
