Program Goals Participants will: 1. Understand and be able to explain the basic tenets of primary palliative care 2. Be able to use and implement strategies to identify appropriate patients that may benefit from palliative care services 3. Understand and demonstrate key communication skills related to helping patients and families clarify goals of care and further advance care planning 4. Understand how the communication framework can be adapted to clinical practice 5. Develop a Quality Improvement (QI) project that employs the above concepts and aims to improve outcomes related to primary palliative care (see below) Please note: This program is NOT specifically designed to help hospitals or hospital-systems develop palliative care consult services or inpatient hospice units. Program Specifics Potential Participants: MDs, NPs, RNs, Physician Assistants (PAs), Social Workers (SWs) or Chaplains (4-5 per hospital); a physician champion is a required member of the team, though the physician need not be the core-team leader.
