Description
In collaboration with the Section of Palliative Medicine in the Division of Hospital Medicine at Northwestern University Feinberg School of Medicine, the contracting hospital will participate in the following training and mentoring.
• Two day training session for quality improvement (QI) teams to learn about the intervention, exchange ideas with other sites and work with their mentor to establish an action plan.
• Focused Training in key communication skills necessary for elucidating a patient’s Goals of Care
• Interactive training utilizing trained actors and role playing
• Use and implement strategies to identify appropriate patients for palliative care services
• Establish and carry out plans for coordinating palliative care and hospice services for their patients.
• 1 year of mentoring by physicians trained in palliative care and goals of care communication. Mentors meet with site leaders approximately monthly via teleconference to help teams tailor their efforts to the needs of their institutions, assist with implementation, and evaluate its impact.
• At least 8 mentor calls will be completed
• On-site visit by faculty mentor
• Mid-year webinar for sharing lessons learned
• Access to the CPC Community
• Repository for related background material
• Listserv where participants share ideas and documents
• Reunion Conference at completion of year-long program.
• Each group is required to present their accomplishments, challenges, and lessons learned
