Description

This proposal intends to identifying then implementing efficient and effective communication between HIV-clinics and community pharmacies delivering comprehensive MTM to persons identified at risk for dropping out of care. Specific aims, developed through the 6-step CDC framework for program evaluation and in partnership with full project team are: (1) identify current best practices for communication between individual HIV prescribers and clinics and Walgreens Center of Excellence pharmacies, (2) design algorithm to allow for maximal efficiency in communication between HIV-clinic and pharmacy and based on individual patient need, (3) assess algorithm for intended effect in the 10 enrolled test sites and, (4) disseminate results across multiple disciplines and venues to allow for broader application of knowledge gained.
