Improving the Health of Illinoisans Through Chronic Disease Prevention

As part of the Funding Opportunity Announcement (FOA) DP18-1815, “Improving the Health of Illinoisans Through Chronic Disease Prevention”, the Illinois Department of Public Health (IDPH) has been tasked with implementing and evaluating evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in high-burden populations/communities within Illinois, contributing to improved health outcomes. Northwestern University’s Center for Health Information Partnerships (CHIP) will collaborate with IDPH to develop strategies focused on the prevention and management of CVD. NU team will contribute to the implementation of one of the five strategies defined by IDPH: to promote the adoption and use of electronic health records and health information technology to improve hypertension and management of adults with hypertension. CHIP researchers will create a cohort for hypertension within the Chicago Area Patient Centered Outcomes Research Network (CAPriCORN) to develop and track clinical quality measures for patients with hypertension. Dr. Theresa Walunas will lead this collaboration effort with the support of key members of a Northwestern team experienced in large scale multi-institutional collaborations.
