Effective and sustainable interventions to reduce cardiovascular disparities are urgently needed. The goals of this K24 are to accelerate the implementation and sustainability of proven cardiovascular disease prevention and treatments from research to practice and community settings, and to train and mentor investigators to conduct community engaged implementation research to decrease cardiovascular health disparities.
