Description

We aim to implement a CIP for IBD focused on structured, step-wise, treating-to-all-targets, including: patient reported outcomes, biomarkers, blinded central endoscopy scoring of mucosal inflammation, nutrition, and mental health well-being. This prospective intervention will combine the Practical Robust Implementation and Sustainability Model (PRISM) framework with Learning Health Care models to assess the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) of our IBD-CIP. The evidence generated would support an expansion and broader implementation of current treat-to-target recommendations which are focused largely on mucosal healing, and future clinical trials across the Chicagoland and neighboring regions through programs such as CAPriCORN, a PCORI funded data warehouse for over 12 million covered lives in norther Illinois.
