Description
With the population of U.S. adults ages 65 and older growing at a rate 3 times that of those under 65, primary care providers must be prepared to care for the 80% of older adults who have complex care needs due to multiple chronic conditions (MCC). Our Northwestern Older Americans Independence Center will infuse geriatrics principles into primary care and align services with patients’ priorities, enhance treatment decision making and promote self-management to optimize function, independence and quality of life among older adults with MCC.
