Description
AIMS:
1. Improve access to primary care and care coordination for individuals with serious and persistent mental illness by evaluating the feasibility of offering primary care services within the Stone Institute of Psychiatry Outpatient Clinic. This is a Northwestern Medicine Community Health Needs Assessment (CHNA) priority goal and is in alignment with best practices in mental health care.
2. Promote healthy lifestyle including counseling on healthy eating, exercise and tobacco cessation.
3. Implement guideline-based screening for common medical disorders in a chronically mentally ill population.
