Use of a simulation-based mastery learning ventilator curriculum to improve care in the medical intensive care unit

Mechanical ventilation is a highly morbid intervention with complications, such as ventilator-associated lung injury and ventilator-associated pneumonia, developing in up to 25% of patients. Management of patients requiring mechanical ventilation is complex and requires familiarity with the intricacies of the ventilator as well as an ability to adapt mechanical ventilation to meet the changing needs of the patient. Simulation-based mastery learning (SBML) research at Northwestern has shown that improved skills after simulation yield improvement in clinical outcomes in advanced cardiac life support, central venous catheter, paracentesis, and thoracentesis procedures. Therefore, our proposed study will be a prospective, randomized trial using a mastery learning model to train residents on the principles of mechanical ventilation. The number of ventilator days and rate of ventilator-associated events between the SBML trained and non-trained residents will be assessed. Ultimately, we aim to refine the mastery learning curriculum for ventilator management to train respiratory therapists, critical care fellows, and advanced practice providers at NMH so that we can assess the effect of team training on patient outcomes in the intensive care unit.
