Description
The PACT project provides tools and skills training to participating providers to help enhance advance care planning practices within their healthcare system. As part of the project hospitals will create project teams, implement PACT processes on a designated unit or patient population, provide opportunities for select clinicians and staff to receive in-depth communication skills training, and form collaborations with key post-acute care partners. To determine the efficacy of the project, participating institutions are required to track process and outcomes data.
