Description
This project focuses on supportive care for Adolescents and Young Adults (AYA) with cancer. Through this project, we aim to operationalize processes to share patient care tools and resources, and connect AYA patients with supportive services identified by screening: psychosocial/distress, practical, family, caregiver support, spiritual, treatment concerns, nutrition, physical concerns, palliative specialty care, etc., and optimize matching of patients’ needs to targeted available resources. In addition, we will trial and expand use of content, tools and techniques developed for the AYA population. The project will also pilot and implement the 4R Oncology Model for Patient Care Sequences for AYA patients focused on quality of life, supportive care, and holistic primary care.
