The Effect of Clinical Reasoning Feedback on Hospital Medicine Physicians

In this project, we will: 1. Describe how often changes in clinical plans occur at transitions. 2. Provide feedback to hospitalists when clinical decisions are changed. 3. Develop a better understanding of the natural history of changes in diagnosis that occur in the context of a modern hospitalization. The project I have envisioned looks to investigate changes in clinical decisions that represent potential errors in clinical reasoning. In our current state, we do not have a method to measure how often diagnostic errors are occurring. Furthermore, we do not have a process in which to improve performance.
