It is well recognized that CAD is the leading cause of heart failure; ischemia from CAD may be either a cause or a consequence of an acute exacerbation. 9 Noninvasive imaging to determine the presence and extent of myocardial ischemia is an important component in the evaluation of patients with heart failure and known ischemic heart disease. The revascularization of patients with hibernating myocardium may improve their chances for survival, cardiac ejection fraction (EF), and heart failure symptoms when the procedure is added to optimal medical management with ACE inhibitors, β-blockers, and statins. 8 This therapeutic strategy will be further investigated in the Surgical Treatment for Ischemic Heart Failure (STICH) trial, funded by the National Institutes of Health.
