In this chapter, we will review the metabolic changes that occur during normal pregnancies and those affected by diabetes as well as review the risks of maternal obesity and hyperglycemia on maternal, fetal, and neonatal outcomes. Management of both preexisting and gestational diabetes in pregnancy will be reviewed in detail including up to date medications and diabetes technologies. Postpartum issues including changes in insulin sensitivity, breastfeeding and contraception for women with preexisting diabetes will be discussed, the importance of postpartum glucose screening and lifestyle modifications for women with a history of gestational diabetes will be highlighted.
