A newborn male presented with three lower extremities, two imperforate ani,two penises, and a single supralevator imperforate colon. The care of this child was complicated by a life-threatening left diaphragmatic hernia. A left colourethral fistula and a right hypoplastic urethra are also documented. This combination of congenital anomalies has not been previously recorded. The successful operative plan and management are outlined.
