N2 - To the Editor: “The Clinical Challenge of Disseminated Intravascular Coagulation” by Daniel Deykin (in the September 17 issue) is a succinct summary of the pathologic physiology of this disorder. However, in the brief section dealing with therapy, we believe that Dr. Deykin has not placed sufficient emphasis on measures other than heparin for the treatment of this syndrome. Although heparin administration is of value in the chronic forms of this disorder (as illustrated by the case of carcinoma reported by Mosesson, Colman and Sherry1), we and others2 have questioned its use in acute forms of disseminated intravascular clotting (DIC),.
