Establishing the optimal maintenance dose of digoxin

Abstract

To determine the relationship between digoxin dose and clinical response, we studied the effects of increasing the maintenance dose for 18 chronic heart failure patients in sinus rhythm who were being simultaneously treated with diuretics and angiotensin-converting enzyme inhibitors. The study encompassed the period of dosage adjustment and a subsequent 12-week randomized, double-blind, placebo-controlled trial. Increasing the maintenance dose of digoxin while maintaining serum concentration within the therapeutic range resulted in a significant increase in left ventricular ejection fraction but no significant decrease in heart failure score, increase in exercise tolerance, or changes in neurohumoral profile.
