Identifying angiographically significant coronary artery disease utilizing albunex® in dobutamine stress echocardiography (DSE)

Abstract

Background: The value of DSE has been well established as a diagnostic test. The use of ultrasound contrast agents for enhancing endccardial borders has also been well documented. We hypothesized that Albunex® used in conjunction with DSE enhances endocardial borders, improves wall motion delineation and improves the diagnostic value in patients undergoing stress echocardiography. Methods: Retrospective analysis of 405 patients undergoing DSE between 01/01/96 and 08/01/96 was performed. Patients with history of prior CABG were excluded. Patients with inducible ischemia on DSE and subsequent coronary angiography were analyzed. Angiographically significant lesions were defined as greater than 70% stenosis in epicardial vessels. Patients with inducible ischemia on DSE were divided into two groups based on the use of Albunex®. Patients were given Albunex® at peak stress during DSE if 2 or more of the 6 endocardial segmente in the apical views were not clearly delineated. Results: 40 patients with inducible ischemia on DSE also had coronary angiography. 14 of 40 patients (35%) received contrast during DSE and 26 of 40 patients (65%) did not receive contrast. ANGIOGRAPHIC LESIONS INDUCIBLE ISCHEMIA ON DSE Contrast Enhanced No Contrast ≥ 70% stenosis 10/14 (71%) 11/26 (42% Conclusions: (1) Albunex® used in conjunction with DSE identified angiographically significant coronary artery disease in 71% of patients as compared to 42% of patients who did not receive contrast. (2) Albunex® may increase the true positive and reduce the false positive rates.
