Abstract
PCFD is a spectrum of diseases resulting in a painful flatfoot which progresses in a complex, but often predictable, fashion. In order to truly understand the deformity, it is imperative to recognize the importance of the medial ligaments of the ankle, specifically the SLC and the superficial and deep deltoid ligament, and their contribution to a neutral hindfoot and reduced mortise. It is through attenuation of these medial structures that the foot can abduct at the TN joint and the hindfoot fall into valgus. With the weakening of these structures, more force is placed through the PTT, in an attempt to maintain the medial arch. When attempting to correct this deformity surgically, it is critical to evaluate and attend to the injured ligaments.
