Abstract

Patients receiving long-term care at home commonly have intervals of hospitalization for acute illness or exacerbations of their chronic disease. Whether the transition between home and hospital is a smooth one depends in part on scrupulous attention to all details of the patient's long-term regimen. For the patient described here, a seemingly insignificant change had serious consequences.
